Definition and epidemiology
Juvenile systemic sclerosis (JSSc) is a rare chronic connective-tissue disease of childhood characterised by fibrosis of skin and internal organs on a background of microvascular injury and autoimmunity. Onset before age 16 accounts for under 5% of all systemic sclerosis; mean onset is around 8–10 years with a female predominance. Compared with adult disease, JSSc more often has diffuse cutaneous involvement but, reassuringly, a lower frequency of severe pulmonary hypertension and renal crisis, and overall better survival — though musculoskeletal and cardiopulmonary morbidity remain significant.
Clinical features
Raynaud phenomenon is the most common presenting feature and frequently predates skin change by months to years. Skin involvement progresses from early oedema to sclerotic induration with sclerodactyly, digital tip ulcers/pitting scars, and contractures; the face may become mask-like with a reduced oral aperture. Systemic features span the gut (dysphagia, reflux, dysmotility), lungs (interstitial lung disease, the leading cause of death), heart (myocardial fibrosis, conduction disease, pericarditis), kidneys (rare scleroderma renal crisis), and musculoskeletal system (arthralgia, myositis, tendon friction rubs). Antinuclear antibodies are usually positive; anti–topoisomerase I (Scl-70) and anticentromere antibodies aid subtyping.
Diagnosis
JSSc is a clinical diagnosis supported by classification criteria and characteristic vascular findings. The 2007 PRES/ACR/EULAR provisional criteria require the one major criterion — proximal skin sclerosis/induration proximal to the MCP or MTP joints — plus at least two of 20 minor criteria across nine systems (vascular: Raynaud, nailfold capillary abnormalities, digital tip ulcers; gastrointestinal; renal; cardiac; respiratory; musculoskeletal; neurologic; and serologic — ANA, SSc-selective antibodies). Sensitivity is ~90% and specificity ~96%. Nailfold capillaroscopy is pivotal: giant capillaries, capillary dropout/avascular areas, and dilatation define the “scleroderma pattern” and, combined with Raynaud and SSc-specific autoantibodies, identify children at risk before overt skin disease. Baseline organ screening (PFTs with DLCO, high-resolution chest CT, echocardiography, and oesophageal/renal assessment) is essential at diagnosis.

Red flags
- New dyspnoea or a falling DLCO — interstitial lung disease or pulmonary arterial hypertension, the leading causes of death.
- Accelerated hypertension with rising creatinine — scleroderma renal crisis (rare in children but an emergency).
- Digital ulcers, blanching, or critical ischaemia — severe vasculopathy needing urgent vasodilator therapy.
- Dysphagia or recurrent aspiration — oesophageal dysmotility.
- Syncope, arrhythmia, or pericardial effusion — cardiac fibrosis or conduction disease.
Management overview
Care is multidisciplinary, organ-directed, and best led with paediatric rheumatology. Active skin and internal-organ fibrosis is treated with immunosuppression — methotrexate, mycophenolate mofetil, or cyclophosphamide for progressive interstitial lung disease, with biologics (e.g. anti–IL-6 or B-cell depletion) in refractory disease. Vasculopathy is managed with vasodilators — calcium-channel blockers first-line for Raynaud, phosphodiesterase-5 inhibitors and endothelin-receptor antagonists for digital ulcers and pulmonary hypertension, and prostanoids for critical ischaemia. ACE inhibitors are the cornerstone of scleroderma renal crisis, and proton-pump inhibitors with prokinetics address gut disease. Photoprotection, physiotherapy to preserve range of motion, nutrition, and scheduled cardiopulmonary surveillance complete the plan. Doses are intentionally omitted; verify against current guidance.
