Definition and epidemiology
Infantile cholestasis is impaired bile formation or flow presenting in the first months of life, defined biochemically by conjugated (direct) hyperbilirubinemia. Per the 2017 NASPGHAN/ESPGHAN guideline, a direct bilirubin >1.0 mg/dL is abnormal and should trigger evaluation — the older “20% of total” rule is no longer recommended. It affects roughly 1 in 2,500 infants. The single most important reason for urgency is biliary atresia, the leading indication for pediatric liver transplant, whose surgical outcome (Kasai portoenterostomy) deteriorates sharply with each week of delay.
Clinical features
The cardinal triad is jaundice persisting beyond the physiological window, acholic (pale/clay) stools, and dark urine (a newborn’s urine should be colourless; dark urine that stains the nappy is always abnormal). Hepatomegaly, a firm liver, splenomegaly, failure to thrive and coagulopathy may be present. Crucially, cholestatic jaundice is frequently mistaken for benign breast-milk or physiological jaundice — but those cause unconjugated hyperbilirubinemia and never pale stools. Any jaundice beyond 2 weeks of age (or 3 weeks in a thriving, breastfed infant with normal stool and urine colour) mandates fractionation of bilirubin.
Diagnosis
The pivotal first step is measuring total and conjugated/direct bilirubin in any infant jaundiced beyond 2 weeks (no later than 3 weeks if breastfed). If cholestasis is confirmed, a structured, parallel workup is warranted because of the time-critical need to exclude biliary atresia: liver function tests (including GGT), coagulation profile, and a search for treatable and life-threatening causes — sepsis, urinary tract infection, hypothyroidism, galactosemia, tyrosinemia and panhypopituitarism. Hepatobiliary ultrasound (looking for a triangular cord sign, absent/abnormal gallbladder), and where indicated hepatobiliary scintigraphy and liver biopsy, evaluate for biliary atresia. A normal ultrasound does NOT exclude biliary atresia — referral must proceed regardless of imaging. Newer stool-colour card screening programmes aid earlier detection.

Red flags
- Acholic stools at any age in a jaundiced infant — assume biliary atresia until excluded
- Conjugated bilirubin >1.0 mg/dL — never observe; always evaluate
- Coagulopathy or bleeding (vitamin K-dependent factor deficiency from fat malabsorption)
- Signs of sepsis, hypoglycaemia, cataracts or genital ambiguity (point to galactosemia, panhypopituitarism)
- Infant presenting at >4–6 weeks with cholestasis — referral is already urgent
Management overview
Cholestasis is a “cannot-miss” referral: send to pediatric hepatology/gastroenterology without delay, ideally before 60 days of life if biliary atresia is suspected, since Kasai outcomes are best when performed early (especially within 30–45 days). Concurrently, treat reversible causes promptly (antibiotics for infection, dietary exclusion for galactosemia, thyroxine for hypothyroidism). Supportive management addresses fat-soluble vitamin (A, D, E, K) deficiency, optimises nutrition with medium-chain-triglyceride-rich feeds, and manages pruritus and complications of chronic liver disease. Give parenteral vitamin K for coagulopathy. In India, where neonatal jaundice is often managed at primary level, the practical safeguard is a simple rule embedded in every well-baby and immunisation contact: check stool and urine colour, and fractionate bilirubin in any infant still jaundiced at two weeks. Treating clinicians should counsel families that prolonged jaundice with pale stools is never normal and warrants same-week assessment, not watchful waiting.
